Trustees Meeting

I, the undersigned Permit Holder, have read and understand the Poland Township Oak Grove Pavilion Rental Policies and Rules on the use of the Poland Township’s Oak Grove Pavilion and agree to comply with the Policies and Rules. I will attend the Event and I understand and agree that I am responsible for the group’s conduct and respect for the facility. I understand that I am responsible for cleaning up the Pavilion and surrounding area before the end of the rental time. I further understand that any property damage to the Pavilion and the surrounding area and any costs for property damage will be charged against me, the Permit Holder.

For and in consideration of the permission to use the Pavilion owned by Poland Township, I, the undersigned, agree for myself, my heirs, executors, administrators, personal representatives, event participants and assigns, to release, acquit, discharge, covenant not to sue, indemnify and hold harmless, Poland Township, the Poland Township Board of Trustees, its employees, officers, servants, and agents of and from any and all actions, causes of actions, claims, demands, damages, loss of services, expenses and compensation, of whatever kind or nature, present or future, which I, myself, my heirs, executors, administrators, personal representatives, event participants and assigns, have or may have on account of, or in any way arising out of, any and all personal illness, injury or property damage which may result from or in any way connected with Poland Township allowing me to use the Pavilion owned by Poland Township. I further acknowledge that I assume all risks of harm, injury, and property damage associated therewith.

POLAND TOWNSHIP OAK GROVE PAVILION RENTAL POLICIES AND RULES

Poland Township’s Oak Grove Pavilion is available for rental for picnics, birthday parties and other similar events.

• The Pavilion accommodates up to 80 people.
• Rental permits are available at the Poland Township Administration Building, 3339 Dobbins Road, Poland, OH 44514 from 9:00am to 3:00pm, Monday- Friday.
• No permit applications will be taken over the telephone.
• Any costs for property damage to the Pavilion will be charged against the Permit Holder.
• Completed Rental Application must be submitted to the Township and a permit will be issued by the Township before the Pavilion can be used.
• Reservations will be made on a first-come, first-served basis.
• Permit applicants must be at least 21 years of age.
• Events must be chaperoned with at least 2 adults for every 25 children or teenagers.
• Pavilion is available to reserve from May 1st through October 31st with the hours of 10:00am to 8:00pm.
• Pavilion has picnic tables, electricity and water available. Portable restroom facilities are also available.

1. NO ALCOHOL, DRUGS, OPIATES, MARIJUANA AND/OR INTOXICANTS are permitted.
2. NO SMOKING OR VAPING is permitted in the Pavilion or on the Playground.
3. The Permit Holder must be present during the event and cannot assign the permit to someone else.
4. The rental is only the Pavilion and the area immediately surrounding.
5. The Pavilion is available for rent from 10:00am to 8:00pm. Each permit is issued only for the specific date and time stated in the Rental Application. All activity, including set up and clean up, must be completed within the stated time frame.
6. All motor vehicles must park in designated parking areas only. No driving or parking is permitted on the grass. 
7. All grill fires must be extinguished at the conclusion of the scheduled rental time.
8. No alterations may be made to any part of the Pavilion, nor shall any item be removed.
9. No confetti, glitter, or similar materials may be used for decorations or thrown in or around the Pavilion. Decorations may not be stapled, nailed, or tacked to the interior or exterior walls of the Pavilion or to any feature on the property (including Playground, trees, bushes, flowers, etc.). Decorations may only be attached by tape, command strips, zip ties, ribbon, or string. Permit Holder is responsible for removal and proper disposal of all decorations during the rental time.
10. Music must be kept at a reasonable volume and should not disturb adjacent areas.
11. Smoke producing products, fireworks, sparklers and the release of wish lanterns is prohibited.
12. Bounce arounds and other recreational inflatables are prohibited.
13. The Permit Holder is responsible for cleaning up the Pavilion and surrounding area before the end of the event. All trash must be placed in trash receptacles.
14. Pets must be on a leash and cleaned up after.
15. Ticket sales, admission fees, sale of food and/or beverages is prohibited.
16. Soliciting donations or collection of money for any purpose is prohibited.
17. The Permit Holder agrees that they will not discriminate against any participants on the grounds of race, religion, sex, sexual orientation, national origin, ancestry, age, handicap, or veteran status.
18. The Permit Holder agrees to release, acquit, discharge and covenant to hold harmless Poland Township, its Board of Trustees, officers, employees, servants, and agents of and from any and all actions, causes of action, claims, demands, damages, costs, loss of services, expenses and compensation, on or account of, or in any way growing out of, any and all personal illness or injury or property damage which may result to any group or organization member as result of the participation in any activity at the property owned by Poland Township
19. The Permit Holder is responsible for the group’s conduct and respect for the facility. It is the Permit Holder’s responsibility to supervise all guests, including children/minors. If any person attending the event abuses Township property or causes a disturbance, the group will be instructed to leave and charges for damages will be assessed. If any group is asked to leave the premises, they must do so promptly and waive any right to a refund of deposit.
